Adults : IV : 1-2g every 8-12 hrs; IM : 0.5-1g every 12 hrs

Children : IM, IV: 50 mg/kg/dose every 8-12 hours (not to exceed maximum adult dosing)

Renal : Dosages adjustment required according to CrCl.
